David, there is no "requirement" that I can think of for a "bathroom" do have a door. For a while in the 1980s or 1990s many developers did not put doors between the master bedroom and the master bathroom, as I recall, those houses did not sell as well as their models which did have such doors - the market place stated the obvious ... bathrooms need privacy, and privacy requires doors.
I suspect that the same would happen with bedrooms (sleeping rooms) as there is nothing to help reduce noise from entering the bedroom where one is trying to sleep - the marketplace would likely reject that for production houses.
A custom built house? if the owner wants to not have any doors on bedrooms (although I cannot think of a reason for doing that), I do not know of a code which requires any interior doors anywhere.
That said, having doors on bedrooms is a known fire safety life saver (providing that the doors are closed when one goes to sleep):
